Please take 15-20 minutes of your time to complete the following New Patient Forms BEFORE your first dental visit with us. This will ensure that we are prepared for your arrival and will help expedite your appointment.

Please plan on being in our office for about 1.5 hours for your new patient visit; we need a little more time this visit to get to know you and see what we can do to make you smile!

If you have X-rays from another dentist, make sure we receive these BEFORE your first visit, or we will need to take a new set when you arrive.

No Show / Cancellation Policy

When we make your appointment, we are reserving a room for your particular needs. We ask that if you must change an appointment, please give us at least 48 business hour notice. This courtesy makes it possible to give your reserved room to another patient who would like it.

There is a charge for not showing up for scheduled appointments. Repeated cancellations or missed appointments will result in a NO-SHOW/LATE CANCELLATION FEE of $100.

We feel that our patient’s time is valuable. When your appointment is made, a room is reserved, your records are prepared, and special instruments are readied for your visit. Except for in the case of an emergency for another patient, you can expect us to be prompt and respectful of your time as well.
